IVF – In Vitro Fertilization

Introduction:

In Vitro Fertilization (IVF) and Intra Cytoplasmic Sperm Injection (ICSI) are advanced assisted reproductive techniques designed to help couples overcome fertility challenges and realize their dream of parenthood. Here, we provide an overview of the IVF process and its detailed steps, as well as information on ICSI.

IVF Process: A Step-by-Step Guide

Step 1: Pre-investigation to Assess Feasibility

  •   Before embarking on the IVF journey, comprehensive pre-treatment investigations are essential to evaluate the suitability of the procedure. These include hormonal blood tests for the female partner, basic semen analysis for the male partner, and common tests such as blood grouping, HIV, Hepatitis B & C, and screening for sexually transmitted diseases for both partners.

Step 2: Long or Short Protocol

  • Following the pre-assessment, the doctor decides between a long or short protocol based on individual circumstances:
  • Long(Agonist) Protocol: Lasting about one month, it comprises two phases - the Suppression Phase with daily subcutaneous injections and the Stimulation Phase with additional injections to stimulate egg production.
  • Short(Antagonist)Protocol: Typically spanning 14 -16 days, it involves daily injections and medications until the eggs mature. Ultrasound scans monitor ovarian response, usually starting after Day 2 of the treatment cycle.

  Step 3: Egg Collection

  •  Based on the doctor's assessment, a date is set for egg collection. This minimally invasive procedure involves removing the female partner's eggs under vaginal ultrasound guidance, with no abdominal incisions.

Step 4: Semen Preparation and Embryo Transfer

1)     The male partner's semen is collected, and the laboratory performs either IVF or ICSI:

  •  IVF: Eggs and sperm are combined in the laboratory.
  •  ICSI: A single sperm is meticulously injected into each egg.
  • Fertilized embryos are then introduced into the female partner's uterus.

Procedures in Detail

Egg Collection:

  • A day care procedure, requiring a half-day hospital stay.
  • Preceded by evaluation by an anesthesiologist a day or two before.
  • Eggs retrieved under vaginal ultrasound guidance are incubated.
  • Most follicles contain mature, healthy eggs, though not all may yield eggs. Rarely, no eggs are found, and appropriate action is discussed with the doctor.

Embryo Transfer:

  •  Involves inserting a speculum into the vagina to access the cervix.
  • The cervix is cleansed, and a catheter is guided into the uterus.
  • Embryos, prepared by the Embryologist, are gently introduced into the uterus through a fine plastic catheter.
  • Ultrasound ensures precise placement, and embryos are released into the uterus.

FAQ

IVF is a fertility treatment method in which eggs are retrieved from a woman's ovaries, fertilized with sperm in a laboratory, and the resulting embryos are transferred back into the woman's uterus to establish a pregnancy.

IVF is often recommended for couples or individuals with specific infertility causes, including fallopian tube blockages, male factor infertility, endometriosis, unexplained infertility, or when other treatments have been unsuccessful.

The typical IVF process involves the following steps:
  • Ovarian stimulation: The woman is given medication to stimulate the ovaries to produce multiple eggs.
  • Egg retrieval: Mature eggs are retrieved from the ovaries using a minimally invasive procedure.
  • Fertilization: The eggs are combined with sperm in a laboratory dish to create embryos.
  • Embryo culture: The embryos are monitored and allowed to develop for several days.
  • Embryo transfer: One or more embryos are transferred into the woman's uterus.

The procedures involved in IVF, such as egg retrieval and embryo transfer, are typically performed under anesthesia or sedation to minimize discomfort. After the procedure, some women may experience mild cramping or discomfort.

Success rates vary depending on factors such as the woman's age, the cause of infertility, and the clinic's expertise. On average, the success rate per IVF cycle ranges from 20% to 40%. Multiple IVF cycles may be necessary to achieve pregnancy.

While IVF is generally safe, there are potential risks and side effects, including ovarian hyperstimulation syndrome (OHSS), multiple pregnancies, and a slight risk of infection. Consult with your healthcare provider for more information on the specific risks associated with your treatment.

The number of IVF cycles to attempt varies for each individual or couple. Many healthcare providers recommend trying IVF for up to three cycles, but this may be adjusted based on specific circumstances.

Maintaining a healthy lifestyle, including a balanced diet, regular exercise, and stress reduction, can be beneficial. Avoiding smoking and excessive alcohol consumption is recommended.

If IVF is not successful after a certain number of attempts, your healthcare provider may discuss other fertility treatments, such as using donor eggs or sperm, or additional diagnostic testing to explore potential reasons for the lack of success.

IVF costs vary widely by location and individual circumstances. The cost typically includes medications, monitoring, the procedure itself, and related services. Some health insurance plans may cover parts of the costs.
